I certify that I do not, to my knowledge, have COVID-19 currently. Nor have I been in contact with or exposed to any known carrier of COVID-19 within the past 14 days.  If I have had the virus, I certify that I have had at least two negative tests 14 or more days before the date of this Waiver; or I have had a positive antibody test. Health care providers have released me to resume regular activity without limit.  I understand that I am attending this NDTC class, ring rental, or AKC event at my own risk and take full responsibility for my health and safety.  I pledge to follow all AKC and specific Club rules and requirements to mitigate any possibility of infecting others or becoming infected. I will also cooperate with any health department and State or Local guidelines with jurisdiction in this area. Nashville Dog Training Club is in no way liable for any present or future COVID-19 infection incurred at any time by any person, in attendance or not in attendance, before, during, or after this date. *

Disclaimer: I hereby release the Nashville Dog Training Club, animal owners, property owners, instructors, spectators, and/or any participants from any and all liability for accidents, injuries, and/or damages to me, my dog, or property caused by participation in any activities sponsored by the NDTC, including the periods of time immediately prior to and immediately after the conclusion of classes so long as the animals are located on the premises of the NDTC or other training area designed by the NDTC. I understand that by participation, I am a joint venture participant and assume all liability for injury to my dog, my property, or myself occasioned by this participation. This waiver will be effective from the date of signing. *
